College Admission Requirements
1. 2x2 ID picture (white background)
2. PSA Birth Certificate (formerly NSO)
3. Baptismal Certificate
4. If SHS Graduate:
a. SHS Diploma
b. SHS Report Card (Form 138) with LRN
5. If College transferee, Copy of Grades or TOR
6. If from school abroad, provide Certificate of Completion of Grade Level.
7. DBC Recommendation Form
